About MTM CAREWhat is I/DD?
Intellectual and developmental disabilities (I/DD) are lifelong conditions — beginning before adulthood — that can affect learning, communication, daily living skills, or independence. With the right individualized support, people with I/DD live full lives in their own communities: working, learning, building relationships, and making their own choices.
Home and Community-Based Services (HCBS) are Medicaid waiver services that make that support possible at home and in the community, rather than in institutions.

How services begin

How services are funded
Most HCBS services are funded through Oklahoma DDS Medicaid waiver programs and coordinated through a DDS case manager. Eligibility and applications are handled by Oklahoma Human Services — see the official Oklahoma DDS pages for how to apply.

How DDS services work
- Apply with the state. Oklahoma Human Services Developmental Disabilities Services (DDS) determines eligibility for waiver services. Apply through the official Oklahoma DDS website.
- A case manager is assigned. Once eligible, a DDS case manager works with your family to build an Individual Plan (IP).
- Choose providers. Families choose from approved providers for the services in the plan — this is where MTM CARE may come in.
- Services are authorized and begin. DDS authorizes services; the provider delivers them and reports back to the team.
We link to official state pages instead of restating eligibility rules, because those rules can change and DDS is the deciding authority.
Talking with your case manager about MTM CARE
- Tell your case manager which service you're interested in (for example, HTS or Transportation).
- Ask whether it fits the current Individual Plan or needs a plan update.
